Принципы

 

Философские первоистоки когнитивно-реструктурирующей терапии сходны с истоками психологии и психиатрии в целом. Между традициями материализма и ментализма всегда была дихотомия. Согласно материализму, человеческое тело, челове­ческий мозг и нервная система — физические объекты в космосе и подвержены всем тем законам механики, которые управляют материальными телами. Они обладают размером, массой и ве­сом, они могут быть восприняты внешними наблюдателями, их можно измерить, и что особенно важно, они работают в строго определенной причинно-следственной последовательности. Ра­дикальный бихевиоризм, медицинская психология и нейропси­хология — все вернулись к философскому материализму Томаса Гоббса и реализму Джона Локка.

Бихевиоральные терапевты, такие, как Каутела, брали бихе-виоральные методы, например погашение, подкрепление и обус­ловливание, и помещали их в мышление субъекта, добавляя приставку «скрытый». Таким образом, сначала в когнитивной терапии мы обращались к методам скрытого обусловливания, скрытой десенсибилизации и скрытого избегания. Будучи скры­тыми, эти техники тем не менее работают в материальном мире с его законами механики и подвержены детерминизму и при­чинно-следственным отношениям.

У второй традиции, ментализма, — столь же длинная и ко­лоритная история. Предпочтение слова «разум» слову «мозг» и выделение таких понятий, как «воля», «выбор», «ответствен­ность», «цель», «знание» и «вера», свойственных этой тради­ции, относятся к феноменам, которых нет в космосе, которые не подвержены законам механики и не видны при внешнем наблю­дении. Опираясь на философию ментализма и идеализма Пла­тона, Джорджа Беркли и Эммануила Канта, другая группа когнитивных терапевтов вышла из гуманистической, роджеров-ской и экзистенциальной школ. Для этих терапевтов свобода выбора, принятие рациональных решений и принятие ответ­ственности за свое поведение — ключевые принципы психоте­рапии.

Когнитивная терапия, как и психология в целом, на протя­жении всей своей истории находилась в ловушке дуализма мате­риализма и ментализма. Проблема согласования двух традиций всегда была одинаковой, а суть проблемы можно выразить воп­росом: «Как может разум влиять на тело?», «Как можно описы­вать физическими терминами, такими, как "нейросинапсы", "химический носитель" и "эндокринная секреция", такие мен­тальные понятия, как "выбор" и "цель"? Как можно объяснить такие физические понятия, как "причина" и "следствие", в тер­минах ментальных понятий, таких, как "выбор", "решение" и "цель"?»

Эта дискуссия — не просто философская абстракция, она фактически переходит в игру, когда терапевт должен доказать, как в зале суда, намеренно ли клиент совершил преступление, осознавал ли он, что делал, вменяем ли он, чтобы пройти суд, или невменяем по причине психоза, сумасшествия, эмоциональ­ной травмы или наркотической зависимости.

Ключевая трудность в объяснении взаимодействия менталь­ного и физического миров была наиболее кратко выражена современным философом Гильбертом Райлом. Он описал проб­лему своей знаменитой фразой, догматом Духа в Машине (Ryle, 1949, р. 15-16). Дух в машине Райла, как воля в чело­веке, — объект нематериальный. У него нет размера, веса или измерения. Он может проходить сквозь стены и двери, может парить над землей, потому что на него не влияют законы фи­зики, такие, как гравитация. Но машина, как человеческое тело, полностью физична, подвержена действию всех зако­нов и сил, которые влияют на все материальные объекты. Как может дух воздействовать на машину? Как наша воля мо­жет влиять на наши действия? Если дух хочет повернуть ры­чаг или нажать кнопку в машине, его бестелесная рука прохо­дит прямо сквозь них, не дотрагиваясь до рычага и не нажимая кнопку.

В когнитивной терапии эта загадка формулируется так: «Как клиенты изменяют свои мысли? Как клиенты начинают верить в одну идею и отвергать другую? Что значит верить! Изменение убеждений — просто вопрос механического повторения и под­крепления, или для того, чтобы изменить такие интернальные образования, как когниции, необходимы выбор и принятие обя­зательств?»

Вопрос о том, как может дух повлиять на машину, относится и к когнитивной терапии. Как клиенты могут изменить свои мысли? Даже если заменить разум на дух и тело на машину или заменить выбор, веру и волеизъявление разума на телесные ней-ротрансмиттеры, корковые и подкорковые зоны мозга, пробле­ма останется той же.

Поскольку дух не может управлять машиной, разве могут клиенты изменять свои мысли? Но клиенты действительно все время меняют свои мысли, поэтому в теории должна быть ошиб­ка, и она есть.

Райл говорит о том, что дихотомии между мышлением и ма­терией не существует, и поэтому нет и проблемы взаимодей­ствия. Человечество не живет в двух параллельных мирах, ма­териальном и психическом, в одном, где человек подвержен влиянию механистических сил и неподвластных воле причин и следствий, и другом, где действуют воля, выбор, цель и ответ­ственность. Два описания — это просто разные точки отсчета для человеческих созданий. Ни одно из этих описаний не явля­ется ни истинным, ни ложным, неверно было бы говорить, что одно полезнее другого, оба они ценны каждый в своей сфере. Когда выписываешь медикаменты тяжело психически больно­му пациенту, полезно посмотреть на физическое — биохими­ческое взаимодействие и всю цепь химических причин и след­ствий. Но когда консультируешь клиента по поводу жизненно важных решений, полезно посмотреть на ментальное — процесс принятия решений, жизненные цели, определение выбора. Выс­казывание Райла лучше всего подытоживает ответ: «Люди — не машины, и даже не ведомые духом машины. Люди — это люди; это тавтология, которую иногда стоит помнить» (Ryle, 1949, р. 81).

Последний пример, который я приведу в этой книге, — самый личный. Он касается того, как я пришел к принятию философии, описанной выше.

Несмотря на годы изучения психологии, а также философии и науки, понимание сложности человеческой природы пришло ко мне не из моих книг, исследований или многих лет учебы. Оно возникло от личного источника — моего отца. Вот как это произошло.

Отца уже давно нет, но я все еще часто о нем думаю. Он был архитектором, из тех, кто любил искусство гораздо больше, чем технику. Он так сильно любил искусство, что завел семейный ри­туал, который мы, дети, ненавидели: каждое воскресенье или почти каждое он заталкивал всю семью в нашу машину, Старушку Бетси, и вез в музей или галерею на новую выставку. Он говорил маме, что это будет хорошо для нас, детей, что это приобщит нас к культуре или что-то в этом духе, но на самом деле он просто сам хотел посмотреть демонстрацию и хотел, чтобы у него была компания.

Детей младшего возраста не очень воодушевляют затхлые му­зеи искусств, и мы.не были исключением, поэтому мы саботирова­ли экскурсии всеми доступными способами, но от одного показа нам не удалось отвертеться. По стране провозили выставку работ Ван Гога, и вот она появилась в Художественном музее Филадель­фии, поэтому в очередное воскресенье нас впихнули в Старушку Бетси и отвезли туда.

Когда мы приехали в музей, большую часть времени я тратил на то, чтобы найти себе интересное занятие, трогая средневековые доспехи и разглядывая самострелы. Когда я не мог больше избе­гать этого, я пошел посмотреть выставку.

Я бродил, глазея на полотна Ван Гога, и они сразу же мне не понравились. Для меня, десятилетнего мальчика, они казались глу­пыми. Цветы не были похожи на цветы, а у земли были такие цве­та, которых никогда не было ни у одного поля. Взятые вместе, полотна казались мне ненастоящими, они не показывали, что видят люди, когда смотрят на мир. Я сделал вывод, что Ван Гог не умел рисовать.

Ближе к концу выставки находилась работа, которая висела на особом, почетном месте. Перед ней собралась группа восторгаю­щихся людей, мой отец был среди них. Мне стало любопытно, что там такого особенного нашли эти взрослые, поэтому я встал за ними и стал смотреть. Это было изображение ночного пейзажа, огромное небо, возвышающееся над маленькой деревушкой. Небо было насыщенного темно-синего цвета, внизу были набросаны очертания деревни. Самым удивительным в картине были звезды.

Это не были светящиеся точки, это были огромные золотые спира­ли, врезающиеся в небо. Они заполняли и поглощали собой все полотно.

Я смотрел на него некоторое время, но отреагировал так же, как и на остальные, — оно выглядело нереальным. Звезды так не выг­лядят, они похожи на световые точки, а не на спирали, и цвет неба был слишком синим, и текстура слишком зернистая. Все это выгля­дело так, как будто было нарисовано лопатой, а не кистью.

Я хотел пойти и найти себе еще какое-нибудь интересное за­нятие, но вдруг на секунду остановился. Мой отец и другие взрослые продолжали восторгаться картиной, и я помню, что по­думал: «Может быть, я не прав; если бы все видели то же, что и я, никто не ходил бы на эти выставки. Но все ходят. Может быть, они видят то, что я не вижу. В конце концов, я всего лишь ребенок. Что я знаю? Если кто-то что-то не понимает, то это, на­верное, я».

Так я стоял и пытался понять, что видел мой отец, копируя его мимику, когда он рассматривал картину. Если он переносил вес на одну ногу, поглаживал подбородок и говорил: «Хм-гм», я делал то же самое. Но это не сработало. Я продолжал думать о том же — небрежная картина, неаккуратная, нереальная, плохо нарисован­ная. Может быть, другие 10-летние дети могли оценить ее кра­соту; может быть, другие мальчики были более чувствительными, художественно одаренными, проницательными или мудрыми, но я не был таким, я был обычным 10-летним ребенком, и я не ви­дел ничего.

Затем мой отец спросил меня, нравится ли мне картина, и я понял, что влип. Если бы я сказал, что думал: «Мне кажется, она тупая», это бы вызвало большие проблемы. Отец и другие взрослые оценили бы меня как глупого, недоросшего ребенка, которого ни в коем случае нельзя было пускать на эту выставку, и что, главнее всего, отец был бы смущен. Он мог рассердиться, сказать: «Черт с этим», — и потащить нас всех домой. Мама бы расстроилась, пото­му что семейный выход не удался, а брат и сестры орали бы на меня за то, что я расстроил маму и папу и испортил им день. Они бы начали требовать, чтобы в следующий раз меня оставили дома, и хотя я ненавидел музеи, мне не нравилось оставаться одному, а без семьи — еще больше. Все это промелькнуло у меня в уме за долю секунды. Неожиданно у меня сорвалось: «Мне нравится... впе­чатляюще... хорошие цвета». Не искушенная критика этой известной картины, но это было лучшее, что я тогда мог сказать. Отец, кажется, был доволен, и день был спасен.

Я надолго забыл о картине, пока я не переехал в Колорадо. Чтобы сбежать от суеты городской жизни, иногда я уезжал один в горы поздно вечером и просто лежал на горном лугу и смотрел на звезды. Около полуночи в горах звезды светятся, как бриллианты, потому что воздух прозрачен, а огни города далеко. В небе тысячи звезд, и кажется, что они окутывают землю. Когда я смотрел на небо на высоком горном переходе, я всегда осознавал, насколько огромен космос и как малы человеческие создания.

Однажды я лежал на своей полянке и смотрел на звезды, как вдруг у меня в голове возникла картина Ван Гога. Вдруг, через 20 лет, я понял, что она означала, почему она так понравилась моему отцу, почему это полотно было настолько известным. «Звездная ночь» Ван Гога ухватывала ощущение момента, чувство, которое я не мог понять, когда мне было 10, но понял сейчас. Ван Гог изоб­разил человеческие эмоции удивления и благоговения перед ноч­ным небом.

Почему картина стала понятной в горах, а не раньше? Многое произошло за те 20 лет с того времени, когда я впервые увидел ее. Я интересовался астрономией, черными дырами, газовой туман­ностью, и необъятность вселенной меня впечатляла. Я занимался философией и много думал о человеческой природе и о нашем месте в космосе, почему мы здесь и как мала наша земля. Поэтому когда я смотрел на звезды, лежа в горах, я смотрел на них другими глазами и видел жизнь совершенно по-другому, чем это было в 10 лет. Фактически это были те же звезды, но ощущались они по-иному. Звезды, которые я видел в горах Колорадо, ощущались как звезды Ван Гога гораздо больше, чем звезды, которые я увидел, когда мне было 10. Это были огромные вращающиеся галактики, состоящие из миллионов звезд и планет и, возможно, кишащие жизнью, а не просто мелкие точки на небе.

Что есть человеческая реальность? Какое отражение неба явля­ется истинным: взгляд 10-летнего мальчика или вид «Звездной ночи»? Какова наша истинная природа, нас и наших клиентов — подчиняется ли она законам механики и детерминизма, с одной стороны, или законам свободы и ответственности, с другой? Когда я был маленьким, я мог сказать: «Звезды — это точки, люди — это люди. Что ты видишь, то и есть». Но став взрослее, думая, зная и чувствуя больше, я осознал: «Звезды — это вселенные, и люди еде ланы из того же материала. Ты видишь то, что твое понимание делает тебя способным видеть».

Человеческая природа не проста. Она существует на многих уровнях, которые постоянно изменяются, передвигаются и развива­ются. Верхний уровень — уровень простой видимости — то, что мы видим, когда смотрим, то, что я видел, когда мне было 10. Нижний уровень — уровень глубокого смысла и понимания — то, что написал Ван Гог, то, что мы чувствуем в горах, то, что мы замечаем о своей собственной природе. Наш опыт проживания в мире создает этот уровень. Астроном видит спиральные галакти­ки, квазары, пульсары, черные дыры и звездную механику. Астро­лог видит созвездия и космические детерминирующие силы, влия­ющие на человеческую природу. Капитан корабля видит меридианы долготы и широты. Священник видит всесотворяющую силу Бога, направляющую человечество. На нижнем уровне нельзя уже гово­рить, что мы видим то, что есть, — для нас существует лишь то, что мы видим.

Итак, какое небо реально и что является нашей истинной при­родой? ■

Все. Все зависит просто от того, как мы на это смотрим.

После многих лет консультирования моих приятелей — челове­ческих созданий я стараюсь не забывать, что человеческая природа многослойна, и я стремлюсь понять тот уровень, на котором нахо­дятся мои клиенты. Я знаю, что некоторые клиенты застряли на поверхности и, чтобы стать счастливее, им нужно опуститься глуб­же. Нет правильных или неправильных уровней, но есть более и менее полезный взгляд на мир. Когда я консультирую клиентов, которые видят только точки в ночном небе, я пытаюсь показать им скрытые за точками вращающиеся многоцветные галактики, танцу­ющие в звездной ночи.
